(b) Deficiencies downgraded to a circled X for more than one
operation may go on the DA Form 2404 DEF MAINT.
However, a set calendar
date for the end of the limited operations must be included. This date will
be no more than one week from the date of the downgrade action. Print "DA
Form 2404 DEF MAINT" after the limits, and add the date when the circled X
comes off the DEF MAINT record.
(c) Deficiencies downgraded to a circled X for a single day or
operation will return to an X status symbol at the end of that period.
(d) Equipment cleared for limited operations is still carried as
NMC on DA Form 2406 and DD Form 314.
(3) Column e - Initials.
The CO or maintenance/motor officer or
higher authority initials for limited operations entries. The person taking
the corrective action or transferring the information initials other
entries. Then, they should complete block 9a.
(4) Blocks 9a and 9b--Signature and Time.
The CO or the
maintenance/meter officer signs first name, middle initial, last name, and
rank in block 9a.
NOTE
A signature in block 9a, like a signature in block 8a,
keeps the DA Form 2404 from being used after the
current dispatch.
A new form must be started for
further operation.
Block 9b, "Time," can be left blank, or used locally as needed.
g. Using the DA Form 2404 for Maintenance Services and Inspections
(figure 6 on the following page).
(1) Block 1 - Organization.
Enter the name of the unit that owns the
equipment.
(2) Block 2 - Nomenclature and Model.
The noun abbreviation and
model of the equipment goes in this block.
For floating equipment, enter
