(5) It records the results of technical inspections on
equipment. When needed, the DA Form 2404 shows serviceability codes listed
in the Army regulations, technical bulletins, or other publications
requiring technical inspections of equipment.

When more than one DA Form 2404 is needed for an inspection or
service, number the pages. Print the page number at the right side of the
form's title block.
If, for example, two DA Form 2404s are used for a
service or inspection, print "1 of 2" on the first page and "2 of 2" on the
second page.
