Introduction
There  is  an  old  saying  that  "the  job  isn't  finished  till  the
paperwork is done."  In the Army this probably is truer than anywhere
else.
When tasked with troubleshooting or repairing a malfunction,
the  Army  (and  your  command  in  particular)  needs  some  way  of
documenting  the  time  spent  troubleshooting,  and  the  time  spent
repairing the vehicle or piece of equipment.
For this purpose, the
Army  uses  a  DA  Form  2404,  equipment  inspection  and  maintenance
worksheet.
The topic of this task is DA Form 2404.  In this task, we will cover
what the form is, its uses and purpose, and how to fill it out.

Equipment Inspection and Maintenance Worksheet (DA Form 2404)

Purpose.
DA Form 2404 has five major purposes.
